  इसका निर्माण सबसे पहले वायरल रक्तस्रावी बुखार इबोला के इलाज के लिए किया गया था।

  इसे अमेरिकी फार्मास्युटिकल गिलियड साइंसेज द्वारा बनाया गया है।

  फरवरी में US नेशनल इंस्टीट्यूट ऑफ एलर्जी एंड इंफेक्शस डिजीज (NIAID) ने SARS-CoV-2 के खिलाफ जांच के लिए रेमडेसिवीर का ट्रायल करने की घोषणा की थी। अब यह ट्रायल सफल होता नजर आ रहा है।

• निर्माण

ऐसे किया जाता है रेमडेसिवीर का निर्माण

• रेमडेसिवीर 100mL जीवाणुरहित, प्रीजर्वेटिव-फ्री आइयोफिलाइज्ड सॉलिड इंजेक्शन है। जिसे 19mL जीवाणुरहित पानी के साथ तैयार कर 0.9% खारे पानी में मिलाया जाता है।

  इसे तैयार करने के बाद 5mL रेमडेसिवीर के मिश्रण के साथ एक 20mL खुराक की शीशी तैयार की जाती है।

  इस इंजेक्शन को 30 डिग्री से नीचे के तापमान पर रखा जाना चाहिए। इसी तरह उपयोग से पहले इसे 2 से 8 डिग्री के तापमान पर रखा जाना चाहिए।

• कार्य क्षमता

कैसे काम करती है रेमडेसिवीर?

• रेमडेसिवीर दवा सीधे वायरस पर हमला करती है। इसे 'न्यूक्लियोटाइड एनालॉग' कहा जाता है जो एडेनोसिन की नकल करता है, जो RNA और DNA के चार बिल्डिंग ब्लॉक्स में से एक है।

कितनी असरकारी है रेमडेसिवीर?

• NIAID ने बुधवार को 1,000 से अधिक लोगों पर किए गए परीक्षण के परिणामों की घोषणा की है।

  इसमें बताया गया है कि सांस की तकलीफ के कारण अस्पताल में भर्ती हुए कोरोना वायरस मरीज प्लेसीबो की तुलना में जल्दी ठीक हो गए।

  इस दवा के उपयोग वाले मरीजों के ठीक होने की रफ्तार 31 प्रतिशत अधिक रही है।

  NIAID का नेतृत्व करने वाले डॉक्टर एंथनी फॉउकी ने भी गुरुवार को रेमडेसिवीर के परीक्षण परिणामों की तारीफ की है।

शिकागो में 125 में से 123 मरीज हुए ठीक

• डॉ फॉउकी ने बताया कि रेमडेसिवीर का इस्तेमाल शिकागो के अस्पताल में भर्ती हुए कोरोना के करीब 125 मरीजों पर किया गया था। इसके उपयोग के बाद 123 मरीज अप्रत्याशित रूप से ठीक हो गए हैं। यह बड़ी उपलब्धि है।

अमेरिका ने दी कोरोना संक्रमितों की उपचार में रेमडेसिवीर के उपयोग की अनुमति

• रेमडेसिवीर के परीक्षण में बेहतर परिणाम सामने आने के बाद अमेरिका के राष्ट्रपति डोनाल्ड ट्रंप ने शनिवार को रेमडेसिवीर का कोरोना संक्रमितों के इलाज में इस्तेमाल किये जाने को मंजूरी दे दी है।

  फिलहाल इसका उपयोग गंभीर मरीजों के उपचार के लिए किया जाएगा। इनमें वो मरीज शामिल होंगे, जिन्हें सांस लेने में ज्यादा दिक्कत हो रही है।

  राष्ट्रपति ट्रंप ने बताया कि इस दवा के इस्तेमाल से संक्रमित मरीज महज 11 दिन में ही ठीक हो रहे हैं।

अमेरिकी सरकार को रेमडेसिवीर की 15 लाख खुराक देगी कंपनी

• रेमडेसिवीर की मरीजों के उपचार में अनुमति मिलने के बाद इसे बनाने वाली कंपनी गिलियड साइंसेज ने मरीजों के उपचार के लिए सरकार को 15 लाख खुराक फ्री देने की घोषणा की है। इससे 1.40 लाख मरीजों का उपचार किया जा सकेगा।

भारत में भी रेमडेसिवीर के इस्तेमाल को लेकर की जा रही है तैयारी

• भारत कोरोना वैक्सीन के लिए विश्व स्वास्थ्य संगठन (WHO) के साथ साझा ट्रायल का हिस्सा है।

  इंडियन काउंसिल ऑफ मेडिकल रिसर्च (ICMR) के डॉक्टर रमण गंगाखेडकर ने पहले कहा था कि भारत ने रेमडेसिवीर के ट्रायल पर नजर बना रखी है और उससे जुड़ा डाटा इकट्ठा किया जा रहा है। यदि यह दवा कारगर होती है तो बड़ी कामयाबी होगी। भारत की आबादी को देखते हुए इसकी कीमत और उपलब्धता पर भी नजर रखी जा रही है।

सूबे में कहर बरपा रहे कोरोना वायरस से निपटने के लिए सरकार के स्तर पर हर संभव उपाए किए जा रहे हैं। गंभीर मरीजों के लिए लेवल-थ्री के बेड बढ़ाने के साथ संसाधन जुटाए जा रहे हैं। महंगे इंजेक्शन का बंदोबस्त किया गया है ताकि मरीजों की जान बचाई जा सके। वायरस लोड खत्म करने के लिए सरकारी अस्पतालों में रेमडेसिवीर इंजेक्शन उपलब्ध कराए गए हैं, जिन्हें अस्पताल प्रशासन मरीजों को निश्शुल्क मुहैया करा रहा है।

बाजार में 4000 रुपये तक का है मिलता

कोरोना के गंभीर मरीजों के इलाज में एंटी वायरल रेमडेसिवीर इंजेक्शन कारगर है। हालांकि इन इंजेक्शन की छह डोज लगती हैं। एक इंजेक्शन की बाजार में कीमत 3600 रुपये से लेकर 4000 रुपये है। पूरा कोर्स लगभग 25-30 हजार रुपये का है। इंजेक्शन महंगे होने की वजह से आर्थिक रूप से कमजोर मरीजों के लिए खरीदना संभव नहीं था। लगातार कोरोना से बढ़ती मौतों की संख्या को देखते हुए शासन ने इन इंजेक्शन को निश्शुल्क उपलब्ध कराने का फैसला किया। हैलट में कोविड हॉस्पिटल के आइसीयू में गंभीर मरीजों को निश्शुल्क इंजेक्शन लगाया जा रहा है। महानिदेशक चिकित्सा शिक्षा की पहल पर उत्तर प्रदेश मेडिकल सप्लाई कॉरपोरेशन अस्पताल प्रशासन को इंजेक्शन की आपूर्ति कर रहा है।

पहले दिन दो इंजेक्शन एक साथ

क्रिटिकल केयर विशेषज्ञ डॉ. चंद्रशेखर ङ्क्षसह ने बताया कि कोरोना संक्रमित गंभीर मरीजों के लिए छह इंजेक्शन का पूरा कोर्स है। पहले दिन दो इंजेक्शन की डोज (200-200 मिलीग्राम) एक साथ लगाई जाती है। उसके बाद 100-100 एमजी के चार डोज लगाए जाते हैं। इसका असर कारगर है, वायरल का लोड भी तेजी से कम होता है। तेजी से मरीजों की स्थिति में सुधार होता है। इंजेक्शन के साइड इफेक्ट भी नहीं हैं।

यह सतर्कता जरूरी

इसमें ध्यान यह रखना है कि लिवर फंक्शन टेस्ट एवं किडनी फंक्शन टेस्ट नार्मल होना चाहिए। एक्सरे में निमोनिया के पैच भी दिखने चाहिए। तभी इंजेक्शन लगाने की सलाह डॉक्टर देंगे।

टॉक्सली जुमेब पर मंथन

टॉक्सली जुमेब इंजेक्शन के इस्तेमाल को लेकर डॉक्टर एकमत नहीं हैं। लखनऊ के संजय गांधी स्नातकोत्तर आयुर्विज्ञान संस्थान (एसजीपीजीआइ) के विशेषज्ञ इस्तेमाल की सलाह दे रहे हैं जबकि ङ्क्षकग जार्ज मेडिकल यूनिवर्सिटी (केजीएमयू) के विशेषज्ञ मना करते हैं। इंजेक्शन की दो डोज लगती है। इसका पूरा कोर्स लगभग डेढ़ लाख रुपये का है। हालांकि इसको लेकर शासन से लेकर मेडिकल कॉलेज प्रशासन तक मंथन चल रहा है।

Why is this medication prescribed?
Remdesivir injection is used to treat coronavirus disease 2019 (COVID-19 infection) caused by the SARS-CoV-2 virus in hospitalized adults and children 12 years of age and older who weigh at least 88 pounds (40 kg). Remdesivir is in a class of medications called antivirals. It works by stopping the virus from spreading in the body.
How should this medicine be used?
Remdesivir comes as a solution (liquid) and as a powder to be mixed with liquid and infused (injected slowly) into a vein over 30 to 120 minutes by a doctor or nurse in a hospital. It is usually given once daily for 5 to 10 days. The length of your treatment depends on how well your body responds to the medication.
Remdesivir injection may cause serious reactions during and after the infusion of the medication. A doctor or nurse will monitor you carefully while you are receiving the medication. Tell your doctor or nurse immediately if you experience any of the following symptoms during or after the infusion: chills or shivering; nausea; vomiting; sweating; dizziness upon standing up; rash; wheezing or shortness of breath; abnormally fast or slow heartbeat; or swelling of the face, throat, tongue, lips, or eyes. Your doctor may need to slow down your infusion or stop your treatment if you experience these side effects.

Other uses for this medicine
The FDA has approved an Emergency Use Authorization (EUA) to allow children weighing 8 pounds (3.5 kg) to less than 88 pounds (40 kg) or children less than 12 years of age weighing at least 8 pounds (3.5 kg) who are hospitalized with severe COVID-19 to receive remdesivir.
This medication may be prescribed for other uses; ask your doctor or pharmacist for more information.
What special precautions should I follow?
Before receiving remdesivir,

• tell your doctor and pharmacist if you are allergic to remdesivir, any other medications, or any of the ingredients in remdesivir injection. Ask your pharmacist for a list of the ingredients.

• tell your doctor and pharmacist what other prescription and nonprescription medications, vitamins, nutritional supplements, and herbal products you are taking or plan to take.
